- Must possess a valid purchase receipt with a survey invitation or store number

Receipt Eligibility Criteria
To qualify for the survey:
- The receipt must be original and unaltered
- Only one survey entry per receipt is permitted
- Survey codes must be entered accurately
- Receipts without survey invitations may not be eligible
- Expired receipts are not valid for participation
Who Is Not Eligible?
The following individuals are not eligible to participate in the MyCFAVisit survey:
- Employees of Chick-fil-A
- Immediate family members of Chick-fil-A employees
- Franchise owners, operators, and business partners
- Anyone attempting to participate using fraudulent or automated methods
- Individuals who violate survey rules or terms

Survey Timing Requirements

Reward Eligibility
